If you are off work sick for seven days or less, you may need to self-certify.
Ask your employer for their relevant form.
Otherwise, you may use the HM Revenue & Customs SC2 form to self-certify.
Please complete the 'Admin or Routine Care Request' form to request a new fit note or an extension.
If you have not seen or spoken to a doctor at the practice about your sickness, or we have no information, such as a hospital discharge letter, you will be directed to submit a form to request an appointment with a doctor first.
