At Burvill House surgery we employ a clinical triage system for appointments. This is to ensure fair and equal access to all of our patients. It enables us to prioritise care to those in most need, and offer greater continuity. We use an online service to take the pressure off the phones, so patients who ring can get through to our team easily.
You can state if you have a preferred clinician, if you prefer face-to-face or telephone consultation, if you need a chaperone, interpreter or have any other access or communication needs.
To request an appointment, submit the ‘I HAVE A HEALTH PROBLEM’ form and this will be read by the Triaging Doctor.
You should receive a same-day response and this may include self-care advice or redirection to a more appropriate service.
Please be aware – you may be asked to attend at short notice.
To request an Urgent Appointment:
Please use the ‘ADMIN OR ROUTINE CARE REQUEST’ form to request routine services, including:
Please be aware the admin form is NOT continuously monitored by a clinician and you should NOT submit urgent requests or health problems on this form.
Whilst we encourage our patients to come to the surgery, where we have the proper equipment and facilities available, we do appreciate this is not always possible.
If you do need a home visit, please call reception on 01707 269091 before 11:00.
Home visits are reserved for housebound patients and patients too ill to visit the practice.
The triaging doctor will determine if the medical condition requires a home visit and how urgently. Please be prepared to provide suitable details to enable this triaging.
